All visitors to Leap of Faith Farms are REQUIRED to sign our "RELEASE, ASSUMPTION OF RISK, AND INDEMNIFICATION AGREEMENT." Please note that it only needs to be signed once and then it is permanently filed. We really, really appreciate your cooperation with this mandatory policy.
This includes everyone even those not riding a horse. Grandma coming to watch your lesson? She needs one. Want to stop by and visit the place? You need one. Please understand that this is a private -- not a public -- facility, and we need to protect all of our boarders as well as ourselves.
Thank you so much, and please note that minors have a separate form that must be signed by a parent or legal guardian, and that same parent or legal guardian must also sign this one. Release Form (Adult)

The "PARENT
OR LEGAL GUARDIAN REPRESENTATION AND WAIVER" must be signed for all minors who step foot on Leap of Faith Farms. Please note that the parent/guardian who signs that Waiver must also complete a Release Form for adults, called a "RELEASE, ASSUMPTION OF RISK,
AND
INDEMNIFICATION AGREEMENT."
